This notice explains the website enquiry form and browser features. It does not describe records used to plan or provide services. If you have a question about those records, contact hello@bloomabilities.ca before sharing them.
What the enquiry form asks for
- Required details: your name, email address and permission for Bloom Abilities to contact you about your enquiry.
- Optional details: a phone number, your preferred way to talk, the service you would like to ask about and a general question.
When you send the form, you ask Bloom Abilities to reply using the contact details you provide. Please do not include medical records, assessment reports, diagnoses or detailed personal health information. The form does not accept attachments.
Website services and saved browser settings
We use Netlify to process the enquiry form. When you send the form, the details you entered go to Netlify for processing. The website also loads its fonts through Google Fonts.
If you change the text size, contrast or motion settings, your browser saves those choices. After the form service accepts your message, your browser also temporarily saves a marker to show that it was accepted. This marker contains no name, email address or message text.
